This connected approach helps ensure smoother transitions between services, clearer communication between care teams, and the right support as patients’ needs evolve.
Healing doesn’t stop when a patient leaves the hospital. Services like Health-E-Quip help patients safely transition home with the equipment and support they need to recover and regain independence, extending care beyond the hospital and into the community.
