Certified Community Behavioral Health Clinic

Horizons Mental Health Center is in transition to become a Certified Community Behavioral Health Clinic (CCBHC).  A CCBHC provides comprehensive behavioral health services while also focusing on physical health and whole person wellness.  

Horizons is committed to offering a high quality experience that is individualize to meet the needs of all of our clientele.  We will be increasing our community crisis services, care coordination, health screenings and much more in order to become the mental health center of choice for our catchment area.
